Not so much an embarassing loss, rather a very unlucky one... I always pay attention when I pve, I've been in lot of 0.0 wars in both player owned space and NPC space. So something external must happen for me to lose a pve ship...
Well this time I was running anomalies in a little crowded system, sanctums/havens were busy so I tried some forsaken hub I think or something like that. Well somebody had been there before and botched the kills, so there were 2 or 3 waves of ships, mostly cruisers and frigs (I was in a raven).
The moment I get tackled by no less than 3 elite frigs (with shields barely holding around 20%), a stealth bomber enters local, somehow finds exactly my anomaly (no combat probes, he had bomb launcher fitted), and bombs/torps me to hell in a couple of volleys =/

This is not a loss but a kill... But it's embarrassing... We caught this carrier in anomaly... And we killed it in perhaps 3 or 4 mins with only the ships in that mail (I didn't even have weapons on the catcher). Fun thing: when it was tackled the rats had already broke through his "shield tank"... And it wasn't even the larger Sanctum, it was the small one!

Uhm... I had that happen to me as well... And didn't seem random at all since not one but TWO guys mwd'ed directly to me. I think it may be an issue due to lag... Perhaps you activated cloak while in warp and they saw you arrive in that spot (the server registered the cloak activation a little later than you thought)? I think that's what happened to me.
Either that or some exploit, though I doubt it... It'd be all the rage with the cries against AFK cloakers :D
(Fortunately, no sad losses to add to the thread for the moment :P)
